Complete all sections of the CMS 116 CLIA Application. Include a list of all tests performed and an estimated test volume on page 3. An email address is essential for timely communication.
Provide director credentials:
Email the completed application to your State Agency.
Upon processing, a confirmation letter should be sent to you within 5-7 business days.
Following application entry into the federal system, a fee coupon will be generated and mailed by a federal contractor.
Upon payment processing, the new CLIA certificate will be printed and mailed via US Mail from the federal contractor. Upon receipt, the laboratory may commence testing and billing for the performed tests.

Certain changes necessitate completion of a CMS 116 CLIA Application, mainly Name, Address, Tax ID, or Phone Changes. To make changes, inform your State Agency and include the following:
Keep in mind that a new CLIA certificate is not issued for changes. Confirmation letters are only sent for director or tax identification number changes. For other changes, an automatic receipt acknowledgment is sent.
